The Care Funnel is designed to enhance your discharge planning by helping your facility identify a provider capable of meeting patients' in-home support needs and equipping them with the resources required to thrive at home. Initiated on-site before discharge, the Care Funnel allows our team to assess the patient’s suitability for agency admission, evaluate the safety of their home environment and existing supports (including identifying any gaps), and determine which non-medical aspects of the discharge plan can be incorporated into their service plan.
This proactive process ensures that the patient’s service plan is ready to be implemented immediately upon their return home, promoting continuity of care and providing peace of mind for both your team and the patient. With the Care Funnel in place, we help facilitate a safe, efficient, and effective transition, supporting optimal outcomes for your patients and streamlined operations for your facility.
Identify Patients Needing Support

Facilities reach out to In Good Faith Home Care to initiate the next steps. Our team promptly coordinates with discharge planners, social workers, or case managers to arrange an on-site assessment for the identified patient.
On-Site Assessment & Agency Enrollment
In Good Faith Home Care conducts a comprehensive and complimentary on-site assessment while the patient is still hospitalized. This assessment confirms eligibility, evaluates care needs, and allows for the seamless completion of the agency’s onboarding process.
Service Activation Upon Discharge
Following discharge, the patient begins receiving care from In Good Faith Home Care according to the frequency and scope outlined in their personalized service plan. This ensures continuity of care, reducing gaps in support and enhancing recovery outcomes.
